    Relevance. Syringomyelia is a disease characterized by the appearance of cavities in the spinal cord. The common opinion is that the course of syringomyelia is slowly progressive, due to the increase of the cavities size. However, sparse data indicate that there are clinical and neuroimaging options for the dynamics of the disease symptoms, which differ from the traditional, including the presence of signs of spontaneous cavity collapse detected by neurovisual examination.

    Objective ― to estimate the dynamics of neurologic and neurovisual parameters of syringomyelia with long-term course.

    Material and methods. The research included 57 patients with syringomyelia with a duration of disease of 15 years or more who were not operated. These patients underwent MRI of brain and spinal cord with morphometric analysis and neurological examination at the early and late stages of the disease.

    Results. Analysis of clinical and MRI indicators at the early and late stages of the disease made it possible to identify 4 main variants of the syringomyelia course: abortive, progressive, resolved and latent.
