Using platelet-rich plasma (PRP) in the surgery of retinal detachment with abruption from dentate line

Retinal detachment with abruption from dentate line is a complicated pathology, often requiring the use of a combined surgical approach ― sclera circular depression, intravitreal surgery with retinotomy, massive endolaser coagulation of the retina, silicone tamponade, suture retinal fixation. The use of platelet-rich plasma (PRP) can significantly simplify blocking of the retinal defect in the retina abruption area after vitrectomy. The technology used for treatment of patients with retinal detachment, complicated by its abruption from dentate line of various lengths, consisted in subtotal vitrectomy 25, 27 Ga, delamination to the periphery and removal of the posterior hyaloid membrane, mobilization of the retina abruption zone by carefully removing the vitreous fibers around the abruption and under the retina, pneumoretinopexy (or gas-air tamponade), and blocking the retinal abruption from dentate line with PRP. Endolaser coagulation of the retina to the center from the retinal abruption zone was not performed. The positive effect was obtained in the majority of patients under maximally sparing surgical intervention: full retinal attachment in the postoperative period and improvement of the visual functions of the operated eye were observed.
