Physical injury is one of environmental factors, which can affect the onset and severity of such diseases as rheumatoid arthritis, chronic juvenile arthritis, Still’s disease, spondyloarthritis, in particular, psoriatic arthritis, gout and pseudogout. The literature review is dedicated to the role of trauma in inducing rheumatic diseases, as well as possible mechanisms of pathogenesis. Factors contributing to posttraumatic arthritis development by inflammatory pathway are discussed, including genetic predisposition, epigenetic changes, mechanobiologic and inflammatory factors. The hypotheses of trauma-induced chronic inflammatory arthritis are reviewed, in particular, interplay with microbiota, “deep Koebner phenomenon” and “synovio-enthesial complex” in psoriatic arthritis, the role of neuropeptides, local and systemic hypoxia due to tissue injury, ischemic reperfusion, exposure of mitochondrial structures and nuclear antigens from necrotized tissue, and cartilage debris adhesion on synovial membrane.
