The article presents statistics of burn injuries, disability reasons caused by burn consequences, localizations of post burn deformities. A clinical case is described of a patient with defects of soft tissues of the lower third of shin and non-supporting surface of foot. The method of surgical treatment is described using the vascularized skin plastics by reverse adipofascial flaps of the lower leg in counterpulsation, with outlining the advantages and limits of using such a method. The effectiveness of this method for the treatment of defects of soft tissues of the lower third of shin and non-supporting surface of foot is connected with the constancy of the supply vessels in the flap; possibility of using a large flap; inclusion of the sural nerve in the pedicle with the intraneural and perineural vessels, supplying the flap with additional blood circulation, and the small saphenous vein providing the sufficient venous drainage from the flap, which significantly reduces the number of necroses.
Key words: reverse adipofascial flaps of the lower leg, defects of soft tissues of shin and foot, vascularized skin plastic.
