 July 1, 2019 marks 150 years of the Republic Psychiatric Hospital named after Academician V.M. Bekhterev. Based on descriptions of district asylums, the article substantiates the necessity of organization of Kazan district hospital and presents its history of creation. The scientific and practical views of the first head of the hospital A.U. Frese determined the goals and principles of Kazan District Hospital functioning. The authors describe the problems faced by A.U. Frese in the first years of the hospital functioning, and his attempts to solve them. Then the authors analyze the principles of L.F. Ragozin who became the director after A.U. Frese. They also analyze the accessibility of the hospital at the end of the 19^th century for all patients referred by Kazan district council.
