Technical capabilities of increasing the efficiency of retinal laser coagulation with the help of a forehead ophthalmoscope at threshold stages of retinopathy in premature babies

Comparative analysis was carried out of the results of treatment of two groups of premature babies with threshold stages of ROP, comparable on gestational age (26-32 weeks), mass at birth (487-1701 grams), stages of ROP (III «+» illness, AP-ROP). RLC in both groups was performed with the help of a forehead ophthalmoscope. In the first group (19 eyes, 11 children) RLC was performed with closure of 75% of the area of avascular retina. In the second group (34 eyes, 17 children) RLC was performed on all area of avascular retina with a scleral depressor. To control of completeness of «closing» of avascular retina, we used the digital imaging system RetCam III. In the first group, after 3 months, positive result was achieved in 77.5% of eyes with III «+» illness, retinal detachment developed only in 3 eyes. In the presence of AP-ROP, only in 12.5% cases retinal detachment did not develop. The overall effectiveness of ROP treatment in the first group was only 47.5%. In the 2nd group 3 months after RLC positive result was achieved in 93% of eyes with III «+» illness. In the presence of AP-ROP positive result was achieved in 18 of 20 eyes (90%). The overall effectiveness of ROP treatment in the 2nd group was 91%. The developed modification of RLC, which includes treatment of 100% avascular retina with the help of a binocular forehead ophthalmoscope, increased the efficiency of treatment of ROP threshold stages to 91%, in comparison with classical RLC method (47.5%).
