Study of capillary blood flow in early post-embolizing period during treatment of uterine myoma

Objective. In this study, changes in capillary blood flow in women with uterine myoma in the early post-embolization period were studied.
Material and methods. A computer capillaroscopy of the nail bed of the 3rd finger of the right hand was performed in 81 women with uterine myoma who were admitted to the gynecology department for treatment with minimally invasive endovascular method ― uterine artery embolization (UAE). Based on the degree of the postembolization syndrome (PS), the patients were divided into three groups: mild severity ― group I (n=36); medium severity ― group II (n=30); heavy severity ― group III (n=15). The study of the microvasculature of the nail bed was used as a model for the study of the microcirculatory blood flow of the body as a whole in patients in the early postoperative period. The study of microcirculation was performed in dynamics before and after uterine arteries embolism.
Results. According to capillaroscopy, the change in microcirculation indices in dynamics was determined depending on the severity of PS. The areal density of the capillary network decreased in all three groups of studied women. The compensatory reaction was the narrowing of the diameters of the arterial capillaries in response to the developing venous stagnation in groups II and III. The increase in the pericapillary zone in all three groups was associated with an increase in the severity of post-embolization syndrome: group I ― by 18.1%; group II ― by 24.8%, and group III ― by 56.3%.
Conclusion. Due to the in vivo study of capillary blood flow in the early postoperative period in patients after uterine artery embolization, it was possible to make timely correction in the treatment regimens, thereby reducing the likelihood of severe manifestations of PS.
