Some neurophysiological aspects of tinnitus

The purpose — to evaluate acoustic brainstem evoked potentials in patients with tinnitus and cranialgia.
Material and methods. 128 patients with tinnitus underwent a comprehensive clinical and instrumental examination, including neurological, vertebro-neurological, otorhinolaryngological, audiological, and neurophysiological assessments.
Results. Analysis of the obtained data indicates that patients with a myogenic component of tinnitus predominantly experience pain syndrome and exhibit modulation of tinnitus perception. In contrast, individuals with a neural component more frequently have hearing loss and comorbid conditions. Auditory brainstem response recordings reveal signs of impaired neural conduction at the ponto-mesencephalic level, as well as evidence of auditory system hyperexcitability in patients with tinnitus associated with cranialgia.
Conclusion. According to the study of acoustic brainstem evoked potentials, patients with somatosensory tinnitus and cranialgia showed increased excitability at the level of the auditory nuclei and the lateral lemniscus.
