Second tumors in children and adolescents (by the data of Onco-Hematology Department of Children’s Republican Clinical Hospital during 20 years — from 1997 to 2016)

Development of the second tumor is one of the severe remote complications of chemoradiation therapy of oncological diseases. It is observed in 2-5% of all treated patients. The article analyzes 8 cases of second tumors in children and adolescents aged 2 to 17. The structure of the first tumors included 77.8% cases of solid tumors and 22.2% of hemoblastoses. The patients were treated with radiotherapy as well as antitumoral drugs with mutagenic properties. The second tumor was diagnosed 6 to 96 months after the treatment completion. The second tumor was an acute myeloblastic leukemia in 50% cases, 2 cases of soft tissue sarcoma, 1 case of goitrous tumor and 1 case of brain tumor. The efficiency of the second tumor treatment depends on the time of its appearance, histological variant and localization, as well as the toxicity of the previous therapy. Out of 8 patients, 2 are alive (25%). The experience of successful treatment of the second tumor is presented as a clinical case.
