Robot-assisted surgery for endometrial cancer in morbidly obese patients

The purpose — to evaluate the surgical outcomes of robot-assisted panhysterectomy with or without pelvic lymphadenectomy in the surgical treatment of patients with morbid obesity.
Material and methods. We conducted a retrospective analysis of the clinical characteristics and treatment of 85 patients with endometrial cancer suffering from morbid obesity. The patients were treated at the Department of Operative Gynecology with Oncogynecology and Day Hospital of Moscow Research Institute of Obstetrics and Gynecology named after Acad. V.I. Krasnopolsky from 2013 to 2025.
Results. Based on individual and group medical records, we analyzed the medical data of 85 patients with endometrial cancer. The average BMI of patients who underwent lymphadenectomy was 48.2 kg/m². Pelvic lymphadenectomy was performed in 17 patients (20.00%). One case of conversion due to bleeding was recorded in a patient with an extremely high BMI (>54 kg/m²).
Conclusion. Robot-assisted laparoscopy is safe for treating endometrial cancer in morbidly obese patients. The robotic system allows for a higher likelihood of complete surgical treatment in patients with morbid obesity.
