  • Results of using laser technologies in the surgical treatment of endometrial polyps in reproductive age

     The purpose — to evaluate the frequency of endometrial polyp recurrence after intrauterine surgical treatment using various methods in patients of reproductive age.

    Material and methods. An applied, single-center, comparative, non-randomized prospective study included patients who underwent intrauterine surgical treatment of endometrial polyps. A total of 120 patients of reproductive age were examined at the Gynecology Department of the Rostov-on-Don «Zdorovye» Clinical and Diagnostic Center. Group I included 40 patients who underwent endometrial polyp removal using monopolar energy; group II consisted of 40 patients who underwent endometrial polyp removal using a holmium laser; group III included 40 patients after endometrial polyp removal using a thulium laser. Recurrence rates were assessed using ultrasound examinations 3, 6, and 12 months after surgery.

    Results. According to ultrasound data, recurrences of endometrial polyps were detected in 8 (20.0%) group I patients. In group II, recurrences of polyps were detected in 2 (5%) cases, and in group III — in 3 (7.5%) cases. Uterine pregnancy occurred during the observation period in 5 (12.5%) group I patients, in 11 (27.5%) group II patients, and in 9 (22.5%) group III patients. Adhesions in the uterine cavity were detected primarily in patients who underwent monopolar resectoscopy. Intrauterine adhesions were detected in 9 (22.5%) group I patients, in 2 (5%) group II patients, and in 3 (7.5%) group III patients.

    Conclusions. Despite the widespread use of laser technologies in modern medicine, these technologies have not found widespread application in practical gynecology. However, the results of our study demonstrate not only a reduction in the recurrence of endometrial polyps in reproductive-age patients but also a decrease in the incidence of postoperative adhesions in the uterine cavity, which is extremely important when treating patients planning pregnancy.
