 Modern cataract surgery is based on the preferential use of minimally invasive technologies of small incisions. Phacoemulsification has become an unconditional priority methodology in cataract surgery. In the elderly patients, cataract surgery has certain characteristics and complexity of treatment. Cataract phacoemulsification in patients older than 85 years can reduce the risk of severe postoperative complications and to obtain satisfactory functional results. The paper presents the results of operations of 147 patients older than 85 years. In the vast majority of cases (91.2%), postoperative course proceeded unresponsively. Inflammatory response to eye surgery was observed in 13 cases. Under the standard antiinflammatory, antibacterial and keratoplastic therapy, inflammatory reaction was stopped within a few days.
