Relationship between the thickness of subepicardial fat and structural-functional parameters of postinfarction myocardium with serum vaspin content

Acute myocardial infarction (AMI) is the leading cause of cardiovascular disease (CVD) mortality. The mechanisms by which subepicardial fat thickness (sEF) influences CVD risk remain a pressing issue. Vaspin is a poorly studied adipokine in patients with post-infarction myocardial remodeling.
The purpose — to identify the relationship between serum vaspin and structural-functional parameters of post-infarction myocardium and sEF thickness.
Material and methods. The study included 64 patients with first-time MI: with (n = 36) and without (n = 28) ST-segment elevation. Vaspin was determined using enzyme-linked immunosorbent assay. The structural and functional state of the post-infarction myocardium was assessed by echocardiography using a VIVID3 system. Statistical analysis was performed using StatTech v.4.10.3.
Results. The vaspin level in patients with ST-segment elevation MI (STEMI) was 0.37–122 pg/ml (median (Me) 2.0 with interquartile range (Q1–Q3) 0.77–2.34), without elevation (NSTEMI) — 00.435–1001 pg/ml (Me 1.35, Q1–Q3) 0.79–2.35). The reliability of differences between the groups was 0.032. Analysis of vaspin in patients who had STEMI and interventricular septal thickness (IVST) (p = 0.043), left ventricular posterior wall thickness (LVPT) (p = 0.006) and left ventricular myocardial mass index (LVMI) (p = 0.036) showed significant relationships. In patients after NSTEMI, a relationship was found between vaspin and sEJ thickness (p = 0.012). In patients with a history of STEMI and type 2 diabetes mellitus (DM), regardless of the presence of arterial hypertension (HTN), vaspin ranged from 0.484 to 2.71 pg/ml; in patients with a history of NSTEMI and DM and HTN, it ranged from 0.757 to 0.879 pg/ml; and in patients with a history of NSTEMI and DM without HTN, it ranged from 0.757 to 0.879 pg/ml.
Conclusion. In patients with new-onset STEMI, a direct relationship was found between serum vaspin levels and myocardial parameters (IVST, LVPW, LVM). Patients with a history of MI and concomitant DM had vaspin values ≤2.71 pg/ml, which may indicate its cardiovascular protective properties.
