     Acute myocardial infarction (AMI) is a leading cause of cardiovascular disease (CVD)-related mortality and disability. Biomarkers can predict the course and outcome of AMI. Vaspin is an adipokine that remains a poorly studied marker in patients with cardiometabolic comorbidity.

    The purpose — to evaluate serum vaspin levels during the complicated acute phase of myocardial infarction in patients with comorbid type 2 diabetes mellitus (DM).

    Materials and methods. Patients with new-onset MI (n = 64) were examined, including 20 patients with type 2 DM and 44 without DM. We assessed serum vaspin levels, their relationship with acute MI complications, and glucose level dynamics.

    Results. Our analysis revealed that patients with AMI had lower serum vaspin levels (<1002 pg/ml) compared to healthy individuals. We also found that patients with type 2 diabetes mellitus had lower vaspin values (0.484–2.71 pg/ml) compared to patients without diabetes (0.374–1001 pg/ml). Increased vaspin in the second group predicted a decrease in glucose levels, compared to glucose levels in the first group. Statistically significant differences were found between vaspin and hospital complications (p = 0.021), particularly ventricular extrasystole (p = 0.017).

    Conclusion. Thus, the study showed a relationship between the level of serum vaspin and type 2 diabetes, as well as the course of the acute period of myocardial infarction in patients with this comorbidity.
