Prediction of oxidative stress in children with congenital heart disease (septal defects)

The redox balance in body cells is of primary importance for their normal functioning. Disruption of this balance has a negative effect on the body, including the cardiovascular system.
The purpose — to determine the relationship between rates of oxygen consumption and redox imbalance in children with septal defects in the long-term period after surgical correction.
Material and methods. We examined 96 patients with septal defects aged 8 to 13 years in the long-term period after surgical correction. All patients underwent a treadmill test according to the modified Bruce protocol. The double product (DP) oxygen consumption at maximal load (maximal DP) and DP reserve were assessed. The level of blood catalase and the lactate to pyruvate ratio (L/P) were determined.
Results. In patients with a normal level of catalase, the average value of maximal DP was 238.7 (32.6) c.u. and the reserve DP was 137.4 (22.3) c.u., which was significantly higher than in patients with a reduced level of catalase — 191.5 (29.8) c.u. and 96.8 (18.2) c.u., respectively. In children with elevated L/P levels, significantly lower values of maximal DP (203.9 (33.1) c.u.) and DP reserve (105.9 (18.2) c.u.) were recorded than in patients with normal L/P values — 245.1 (32.1) c.u. and 144.7 (22.9) c.u., respectively. It was established that at a maximal DP level of less than 212.4 c.u., a decrease in the level of catalase can be predicted, and the level of maximal DP less than 212.2 c.u. predicted an increase in L/P.
Conclusions. Oxidative stress in children with septal defects in the long-term period after surgical correction can be predicted by indicators of the maximal double product and/or DP reserve determined during the treadmill test.
