     Objective. Analysis of modern publications on silent myocardial ischemia on the background of arterial hypertension.

    Material and methods. The analysis of modern publications devoted to the study of painless myocardial ischemia in arterial hypertension.

    Results. The absence of a pain signal in coronary myocardial ischemia, unlike angina pectoris attacks, can be a significant cause of the development of severe complications and a fatal outcome in hypertension. The relationship between blood pressure and the risk of developing silent myocardial ischemia in patients with coronary artery disease is non-linear, the highest risk is noted at 142/93 mm Hg. and higher. Developing left ventricular myocardial hypertrophy increases the incidence of painless myocardial ischemia and its severity.

    Conclusion. Silent myocardial ischemia is an important clinical problem. Its presence determines the high incidence of complications of ischemia, its more severe course and unfavorable prognosis. Further study of the relationship of hypertension with painless myocardial ischemia, its course and treatment features is required.
