The article presents the clinical case of ischemicstroke secondary to the dissection of the posterior inferior cerebellar artery (PICA) in a young man. The clinical pattern, diagnostics and nature course of the disease are described. MRI has shown the hypointensiveness in the area of PICA on T2*GRE, and that strongly supported the diagnosis. Proper diagnosis of the PICA dissection is important to predict and manage probable hemorrhagic complications of this disease.

Key words: dissection, posterior inferior cerebellar artery, ischemic stroke.
