  •  Possibilities of predicting coronary artery re-stenting in type 2 diabetes mellitus

     Cardiovascular diseases remain the leading cause of death worldwide. Diabetes mellitus-induced alterations in the vascular wall contribute to a higher incidence of recurrent ischemic events, including during coronary angiography.

    The purpose — to assess the potential for predicting the need for repeat stenting of coronary artery in patients with type 2 diabetes mellitus.

    Material and methods. We analyzed 1,733 medical records of patients with ST-segment elevation myocardial infarction (STEMI) (n = 797 (46%)) and non-ST-segment elevation myocardial infarction (NSTEMI) (n = 936 (54%)). Among these, 386 individuals (22.3%) had type 2 diabetes mellitus.

    Results. The analysis of coronary artery (CA) lesions depending on age showed statistically significant differences (p = 0.001). The study revealed a significant correlation between the frequency of recurrent myocardial infarction and the presence of type 2 diabetes. The odds ratio for recurrent myocardial infarction in patients with type 2 diabetes was 1.77 times higher than in those without diabetes, with a statistically significant difference (95% CI 1.330–2.355). When assessing the risk of multivessel lesions in patients with type 2 diabetes, the odds ratio was 1.584 times higher than in those without diabetes. The odds ratio was statistically significant (95% CI 1.255–2.000). The analysis of predictors revealed that the risk of recurrent myocardial infarction in patients with type 2 diabetes was 1.691 times higher, and the risk increased by 1.022 times with increase of age. When the predictors were assessed, the likelihood of re-stenting in patients with type 2 diabetes increased by a factor of 1.491.

    Conclusion. The study demonstrated a correlation between type 2 diabetes and age with the multivessel coronary lesions, frequency of recurrent myocardial infarction, and the number of repeat CA stents. The findings allow creating predictive models that estimate the probability of repeat stenting based on the presence of type 2 diabetes in patients and the likelihood of recurrent myocardial infarction based on the presence of type 2 diabetes and age.
