  • Place of robot-assisted surgery in the treatment of infertility associated with colorectal endometriosis

    The purpose — to study the practice of robot-assisted surgical interventions for colorectal endometriosis performed in the Department of Operative Gynecology with Oncogynecology and Day Hospital of Moscow Research Institute of Obstetrics and Gynecology named after Acad. V.I. Krasnopolsky.

    Material and methods. The patients were divided into four groups: group I (n = 48) — patients who underwent «shaving» of the endometriotic nodule from the colon surface; group II (n = 36) — patients who underwent discoid colorectal resection; group III (n = 44) — patients who underwent colorectal segmental resection; group IV (n = 28) — patients who underwent colorectal segmental resection using the Natural Orifice Specimen Extraction (NOSE) technique.

    Results. Statistically significant differences (p < 0.0125) were noted between the duration of surgery in group III (212 (162–277) minutes) compared to the others, due to the laparotomy step during intervention. At the same time, the pregnancy rate (PR) was maximum in groups III and IV — 68.2 and 70.8%, respectively (p < 0.0125). Also, these groups were characterized by a lower frequency of disease recurrence: 6.6 and 5.9% in groups III and IV, respectively (p < 0.0125).

    Conclusion. Robot-assisted surgery has unconditional advantages in the surgical treatment of complex gynecological diseases, primarily different forms of deep infiltrating endometriosis.
