The article discusses a clinical case of pain in the lower back due to pathological fractures of lumbar spine against the background of severe secondary osteoporosis induced by chronic obstructive pulmonary disease. Timely diagnostic examination and consultations with relevant specialists made it possible to identify the cause of back pain, prescribe medication for pain, prevent pathological spinal fractures associated with osteoporosis, and provide medical rehabilitation.

Key words: back pain, osteoporosis, vertebral compression fracture, densitometry, COPD.
