Forging a common approach to rehabilitation and habilitation of premature children is one of the crucial tasks at all steps of nursing. For the first time, the rehabilitation potential has been determined on the grounds of clinical and instrumentation data reflecting the damaged structure, functions, activity and participation from the point of the International Classification of Functioning, disability and health for children and teenagers at the time of their transfer from the first or second stage of rehabilitation. It was found that most of the patients have high and medium rehabilitation potential; patients with very low rehabilitation potential have not been identified. Evaluation of rehabilitation potential has allowed determining the principle of organization of a multidisciplinary team of experts, to define the targets of rehabilitation and habilitation, to identify the key expert in the team, and thereby to increase the effectiveness of the rehabilitation activities being held.
