Organ preservation operations with placenta accreta

Objective ― to analyze the experience of organ-preserving operations with true placenta increment.
Material and methods. In 2018, 6 organ-preserving operations were performed in pregnant women with a diagnosis of placenta accreta. The patients underwent a full examination, including a control ultrasound and MRT, each of them had a burdened obstetric-gynecological history.
Results. All operations were performed under inhalation anesthesia with artificial ventilation of lungs; the article provides a complete analysis of the anesthetic provision. The mean operative intervention time was 110 minutes, the average blood loss was 3200 ml. During operative delivery, intraoperative reinfusion of erythrocytes was performed in combination with fresh frozen plasma and erythrocyte mass. In the postoperative period, the method of accelerated recovery was practiced. The postoperative period was uneventful. Women were discharged maximum 13 days after surgery in a satisfactory condition with a child.
Conclusion. With careful assessment of the state of the pregnant woman and an adequate choice of delivery method under placenta location anomalies, is an outcome for the mother and the fetus is quite favorable.
