  • Optimization of surgical access for bypass arterial reconstructions to the tibial artery

    The results of using of the original surgical access (application for invention № 2015142841) to the anterior tibial artery in 36 patients with obliterative peripheral arterial disease in the stage of critical ischemia. According to angiography, the anterior tibial artery was the only inflow artery to perform revascularization. The bypass conduit was made with autovein by «in situ» methodology ― 20, reverse autovein ― 14, PTFE-prosthesis ― 2. Primary patency of bypass grafts was 91.7%, secondary patency ― 100%. Patency of grafts up to 6 months after the intervention was 97.1%. The proposed surgical access to the anterior tibial artery significantly reduces surgical trauma, creates conditions for the reliable protection of the shunt with tissues, creates hemodynamically favorable location, creates conditions for revision of the leg arteries. The proposed access for the isolation and bypass surgery of the only passable anterior tibial artery may be recommended in routine practice.
