    The article describes anatomy of the thumb muscles innervation, and biomechanics of the thumb. The history of opponentoplasty development is described. Critical points in opponentoplasty are described. Indications, advantages and disadvantages of different types of opponentoplasty with denervation atrophy of thenar muscles group are defined.
