New possibilities for early diagnosis of hilar cholangiocarcinoma using the TurboHawk catheter for directed biopsy

Early diagnosis of hilar cholangiocarcinoma is still a challenge for modern medicine. We have studied the diagnostic results of hilar cholangiocarcinoma with the TurboHawk catheter for directed biopsy. In 2014-2016 20 patients of A.V. Vishnevsky Institute of Surgery with suspected hilar cholangiocarcinoma underwent biopsy of bile duct tumor with the TurboHawk device; 6 patients (30 %) were found positive for cholangiocarcinoma and underwent liver and bile duct resection with the formed hepaticojejunostomy. 9 patients (45 %) had unresectable tumor and underwent external-internal or external drainage of bile duct followed by chemioradial therapy. 5 (25 %) patients were found negative for cholangiocarcinoma and stayed under dynamic observation. The method sensitivity was 100 %, specificity — 89 %, and prognostic value — 92 %. Thus, the directed biopsy with the TurboHawk catheter is a high-precision method and allows to substantiate the hilar cholangiocarcinoma with 92 % reliability. However, the diagnossticaccuracy has to be proved by further research.
