New method of treatment of congenital hip dislocation in adolescents and adults

Introduction. Despite actively introduced modern technologies, there is still quite a large percentage of failures after the treatment of congenital hip dislocation (CHD), especially among adolescents. The patients complain of lameness and pain syndrome both in the hip joints and the spine and other joints. Radical intra-articular interventions lead to early coxarthrosis and disability. Endoprosthetics of the hip joint in adolescence is recommended only in the terminal stages of coxarthrosis. In fact, today there is no optimal treatment for adolescents and young adults with congenital hip dislocation.
The research objective was to develop an effective method of hip stabilization in adolescents and young adults with does not interfere with congenital hip dislocation by preserving own bone structures.
Material and methods. The biomechanic substantiation of the new method of surgical correction of the proximal part of femoral bone is presented. The primary analysis is given of the surgical treatment of 66 patients aged 13 to 30 y.o. by creating an additional point of support under the pelvis 10-15 years after the operation according to the authors’ method. The essence of the technique lies in the angulation of the lateral oblique osteotomy of the thigh, which made it possible to extend the limb simultaneously to 4 cm and restore the biomechanical axis of the limb.
Result of treatment was primarily studied in 56 patients. It confirmed the authors’ hypothesis that the rhythm of walking can be improved, the unipolar time increased, and the load on the limb stabilized. The vast majority of patients achieved stabilization of the hip joint, eliminating or reducing the Trendelenburg symptom.
