    In order to study the correlation between specific biochemical markers of neurotrophic and neurodegeneration in patients with primary open-angle glaucoma on the background of neuroprotective therapy, a tear fluid was studied in 23 patients (46 eyes) with primary open-angle glaucoma of the second and third stages, the average age of which is 66.3±10.8 years. For monitoring, a group of healthy individuals was recruited. Along with local antihypertensive therapy in comprehensive treatment of patients with POAG, a complex of peptides isolated from retinas of cattle ― Retinalamin® (GEROPHARM LLC, St. Petersburg) №10 was intramuscularly injected at a dose of 5 mg a day. Before and after treatment in tear fluid, the concentration of a neurotrophic brain marker (brain-derived neurotrophic factor or BDNF) and a neurodegeneration protein — neuron specific enolase (neuron specific enolase or NSE) was localized through the enzyme-linked immunosorbent assay (ELISA) with subsequent calculation of their ratio BDNF/NSE. As a result of the research, it was concluded that with POAG there is a change in the level of the neurotrophic factor of the brain and a marker of neurodegeneration ― neuron-specific enolase in a tear fluid with respect to the control towards the prevalence of neurodegeneration markers in 38% of patients with stage II and in 45% in stage III of the disease. After the use of Retinalamin® in the complex therapy for patients with stage II of POAG, a decrease in initially high concentrations of BDNF/NSE in the SS to the values of the control group was noted. The ratio of neuronal markers of neurotrophy and neurodegeneration reflects the possibilities of compensatory-adaptive and pathological reaction of the eye in conditions of glaucomatous process.
