    The work objective was to examine the incidence, clinical features, diagnostic difficulties, and outcomes of myocardial infarction (MI) in patients with infective endocarditis (IE). A retrospective analysis of hospitalized patients (n=262) with a significant native valve IE was fulfilled. MI was diagnosed in 14 patients (5.3%), more often — of anterior localization (11 patients), found only in males, with left-sided IE, mainly in the onset of the disease and caused a diagnostic error in 10 patients. We marked a great expression of hypertrophy and dilatation of the left ventricle, the magnitude of pulmonary hypertension, severe heart failure, more often the intracardiac destructions (perforation and detachment of folds) (p<0.05), and a two-fold increase in mortality in patients with MI in IE than in those without MI. (43% vs 19%, p<0.05). The possible mechanisms of myocardial infarction and diagnostic difficulties in IE were discussed in comparison with literature data.
