Modern features of the diagnosis of multiple primary malignant neoplasms with lesions of gynecological organs

Objective ― to study the informativeness of magnetic resonance imaging in the diagnosis of lesions of female reproductive organs in the structure of multiple primary malignant neoplasms (MPMN).
Material and methods. 179 patients with polineoplasia with lesion of female genital organs, who were in Autonomous Institution «Republican Clinical Oncological Dispensary» of the Ministry of Health of the Chuvash Republic from 2012 to 2016, were examined. All patients underwent the researches as follows: magnetic resonance imaging (Excelart Vantage Atlas «Toshiba», Japan, 1.5 T), ultrasound (Aixplorer «Supersonic Imagine», France; Ascuvix-V10 and Ascuvix-V20 «Samsung-Medison», South Korea), radiological (Clinоmat and Clinodigit «Italray», Italy, X-ray computer (Light-Speed VEX Plus and Light Speed RT 16, «GE», USA). The reliability of the results is confirmed by the data of morphological verification of the material under study. Differences were considered significant at p≤0.05.
Results. Metachronous polyneoplasia with lesion of female genital organs was diagnosed in 152 cases (84,9%), synchronous ― in 27 cases (15,1%). The structure of the metachronous tumor lesion of the female genital organs during MRI was as follows: endometrial damage ― 69 cases (45.4%), cervix ― 35 (23%), ovaries ― 38 (25%), vagina ― 4 (2.6%), vulva ― 6 cases (3.9%). Synchronous tumors with lesions of the endometrium and ovaries were diagnosed in 11 cases (40.7% each), and of the cervix ― in 5 cases (18.5%). The informativeness of MRI in the diagnosis of synchronous MPMN with the defeat of gynecological organs was as follows: sensitivity ― 92.3%, specificity ― 89.3%, accuracy ― 90.7%; metachronous MPMN, respectively ― 96.7%, 95.5%, 96.1% (p<0.05).
Conclusion. The results of the use of different modes of MRI in the study of the pelvic organs showed that the use of multiparametric MRI is necessary when assessing the structure of gynecological organs and can be used as a method to improve the diagnosis of primary multiple malignant tumors.
