    Metachronous metastasis of kidney cancer to the contralateral adrenal, mimicking the multilocular cyst,, in patient with multiple myeloma and previously operated for kidney cancer and metastasis to the contralateral adrenal is discussed. Morphological criteria and immunohistochemical markers helped to identify metachronous metastasis to the contralateral adrenal mimicking cyst.
