The article presents a substantiated technique of intraoperational prevention of piriform muscle syndrome in treatment of old fractures of the rare column of cotyloid cavity, consisting of transposition of the crossed muscles — short external rotators of a hip during their myorrhaphy (Patent No. 2624387-2017). This prevents compression of sciatic nerve when it comes out of the pelvis minor in sub-piriform area; moreover, in cases of old lesions the short external rotators of a hip already have fibrous changes.
