    The purpose was to investigate the 24-hour ambulatory blood pressure monitoring (ABPM) parameters in adolescents with essential labile arterial hypertension (AH) and obstructive sleep apnea syndrome (OSAS).

    Material and methods. The study included 65 adolescents (21 boys and 44 girls) aged 12 to 17 years with newly diagnosed essential labile arterial hypertension, with a mean age of 13 ± 0.42 years. The patients were divided into two groups: the main group (group 1, n = 36) consisted of adolescents with both AH and obstructive sleep apnea syndrome, while the control group (group 2, n = 29) included subjects with AH but without OSAS. All patients underwent general clinical examination and 24-hour ambulatory blood pressure monitoring (ABPM). During respiratory screening with two-channel pneumography, the apnea-hypopnea index (AHI) was assessed.

    Results. The results of our comparative analysis demonstrate that adolescents with primary labile arterial hypertension complicated by obstructive sleep apnea syndrome exhibit statistically significant higher levels of systolic, diastolic and mean arterial pressure throughout the entire 24-hour period compared to hypertensive patients without OSAS. The most pronounced differences were observed during nighttime hours, indicating impaired physiological blood pressure dipping during sleep and confirming the pathogenic role of apnea in circadian blood pressure rhythm dysregulation.

    Conclusions. The findings underscore the importance of targeted screening for OSAS in adolescents with labile hypertension, as timely diagnosis and management of this condition may lead to improved blood pressure control and reduction of long-term cardiovascular risk.
