Impact of menopausal hormone therapy on the liver

Menopausal hormone therapy (MHT) remains the most effective treatment for vasomotor symptoms of menopause, genitourinary syndrome, and the prevention of postmenopausal osteoporosis. Liver plays a key role in the sex hormones metabolism, making the issue of MHT hepatobiliary safety critically important when choosing therapy.
The purpose — to summarize current literature data on the impact of menopausal hormone therapy on the liver and possible adverse effects.
Material and methods. A systematic search and analysis of publications in international peer-reviewed journals indexed in PubMed, JAMA, EMBASE, and Google Scholar, as well as relevant national scientific journals and clinical guidelines from 2013 to 2026.
Results. Modern data have fundamentally changed the view on the hepatobiliary safety of MHT. Oral forms are indicated for compensated chronic liver diseases, metabolic syndrome, including non-alcoholic fatty liver disease. Transdermal MHT is the drug of choice for primary biliary cholangitis and can be used in non-alcoholic fatty liver disease.
Conclusion. An individual approach, based on assessing liver function before prescription and choosing an appropriate administration route, makes MHT a safe and effective method for the majority of peri- and postmenopausal women.
