The article is devoted to the clinical observation of a patient with hypertrophic cardiomyopathy, left ventricular outlet obstruction, and cardiac arrest during pregnancy. Despite absolute contraindications for prolongation of pregnancy: functional class IV, chronic heart failure 2A, relative gradient in the outlet of the left ventricle at 13 weeks of pregnancy, severe cardiac arrhythmia (paroxysmal atrial fibrillation, tachysystolic form, paroxysmal supraventricular tachycardia with spontaneous restoration of heart rhythm, paroxysm of ventricular tachycardia), pregnancy was prolonged (the patient and her husband refused from interruption). At 30 weeks, the patient was transferred to a maternity hospital specialized in cardiovascular pathology, with the participation of an obstetrician-gynecologist, anesthesiologist, cardiologist, and neonatologist, and was delivered by cesarean section successfully for mother and fetus.

Key words: hypertrophic cardiomyopathy, left ventricular outlet obstruction, cardiac arrest, pregnancy, absolute contraindications for prolongation of pregnancy.
