The article presents a case report of a successful surgical treatment of recurrent chronic sinusitis, where the previous external access frontal sinus surgery, performed in 2012, did not lead to the desired result. The frontal sinus surgery was performed by endoscopic endonasal surgery with 3D CT and under the control of image guidance. The purpose of reoperation was to form a broad frontal anastomosis allowing adequate ventilation and drainage of the frontal sinus in order to reduce the recurrence and complications. The technique reduces the trauma and significantly shortens the in-hospital period of the patient.

Key words: chronic frontal sinusitis, endoscopic endonasal frontal sinus surgery, electro-magnetic image guidance.
