Features of treatment of vasculitic neuropathies

Systemic vasculitis is a heterogeneous group of diseases characterized by inflammation and damage to blood vessels of various calibers. This nosology is divided into separate (primary) diseases and those associated with other diseases, exposure to drugs or toxic agents (secondary vasculitis). In all these cases, primary and secondary systemic vasculitic neuropathy may be identified. If the symptoms of vasculitis are limited to symptomatic lesions of nerves and muscles, the nosology is defined as non-systemic vasculitis. Treatment includes corticosteroids, cytotoxic agents, intravenous immunoglobulins, plasma exchange. It requires a multidisciplinary approach with the participation of neurologists, rheumatologists, physiotherapists, to evaluate the degree of organic damage, the nature of systemic vasculitis, the presence of a viral infection, and the clinical response to treatment of vasculitis neuropathy. As neurologists play an important role in the diagnosis and treatment of patients with vasculitis neuropathy, they should be informed of the classification of this nosology, schemes of treatment, variants of the course of neuropathic disorders in patients with rheumatologic profile. It is necessary to study the promising new methods of treatment for vasculitis neuropathy.
