Experience of external obstetric rotation in breech presentation

The purpose — to analyze the outcomes of external obstetric rotation operations for breech presentation.
Material and methods. In 2024, 21 attempts at external obstetric rotation were carried out at the Kirov Regional Clinical Perinatal Center. The research materials were processed using the Statistica software.
Results. In 2024, 21 attempts at external obstetric rotation were carried out. Of them, 15 were successful, which is 71.4%. The remaining 6 obstetric rotations (28.6%) were considered unsuccessful and the women were delivered by cesarean section. Of the 15 successful rotations, one patient (6.7%) underwent a reversion from cephalic to breech presentation within 24 hours; the rest gave birth through the vaginal birth canal in a cephalic presentation of the fetus. There were no significant differences in the groups of women with successful and unsuccessful obstetric external rotation. According to our results, the rotation effectiveness did not depend on the woman’s age, gestational age, parity, placenta location, the presence of concomitant somatic pathology, or fetal weight.
Conclusion. It is advisable to use external obstetric rotation of the fetus during breech presentation in the practice of an obstetrician-gynecologist. This operation is successful if the indications, contraindications, conditions and technique of its implementation are observed. The use of obstetric rotation is one of the factors in reducing the frequency of cesarean sections.
