Early diagnosis of ankylosing spondylitis

Diagnosis of ankylosing spondylitis (AS) in actual practice is delayed by 7-8 years. This situation is connected with the scarcity and variability of clinical manifestations in the beginning of the disease, poor knowledge of disease symptoms by primary care physicians, late development of radiologic defined sakroiliitus — an important criteria for diagnosis. Use of magnetic resonance imaging (MRI) helps in diagnosis of AS in the early, pre-radiologic stages. At the beginning of the disease there is the appearance of the first clinical signs of AS, among which the main is inflammatory back pain by criteria of ASAS experts in 2009; peripheral arthritis, enthesitis, uveitis, psoriasis, internal lesions also happen. Educational programs for primary care physicians and performance of special diagnostic procedures for suspected AS can timely refer patients to a rheumatologist. Modified AS criteria can be used for diagnosis in the early stages of the disease.
