The article presents the results of treatment of 331 patients, 15 to 78 years old, with widespread peritonitis with varying degrees of severity. Drainage of abdominal cavity was performed to 192 (58 %) of the patients, open abdomen — 11 (3.3 %), programmed relaparotomy — 68 (20.6 %), laparoscopic sanation of abdominal cavity — 60 (18.1 %). The authors propose the integral estimation of peritonitis severity, which allows to determine the method of the primary operation completion. The efficiency of gradual sanations of the abdominal cavity for diffuse peritonitis is shown, if the peritonitis severity does not exceed 29 points by Mannheim peritoneal index.
