We present the clinical case of amyotrophic lateral sclerosis (ALS). The difficulty of diagnosis was related to non-specific initial presentation of the disease, and inaccurate interpretation of the available clinical and instrumental data, which led to late diagnosis and unnecessary surgical treatment.

Key words: amyotrophic lateral sclerosis, diabetes mellitus.
