Complex treatment of the scapulohumeral periarthrosis

Scapulohumeral periarthrosis («frozen shoulder», «stiff shoulder») is one of the most common causes of the persistent pain syndrome and limitation of the shoulder joint articulation. The disease is characterized by pain, especially at night, limitation of the amplitude of shoulder joint movements, and is a frequent cause of outpatient treatment by a neurologist.
Objective ― to determine the effectiveness of complex therapy including a course of blocks of the shoulder girdle muscles in cases of scapulohumeral periarthrosis.
Material and methods. The paper presents the results of complex treatment of 44 patients with scapulahumeral periarthrosis (SHP), including a course of intramuscular blocks of the shoulder girdle muscles. It was found that shortening of the subscapular muscle is the main cause of SHP and the chronic pain syndrome. The therapy effectiveness was assessed using a Croft Shoulder Disability Questionnaire and the visual analogue scale (VAS).
Results. The complex therapy, including a course of blocks of the shoulder girdle muscles, resulted in reduction, sometimes disappearance, of the pain syndrome intensity, and greater amplitude of active and passive movements of the shoulder joint in all patients of the study group.
Conclusions. The subscapular muscle is the source of the pain syndrome in cases of scapulohumeral periarthrosis. A course of intramuscular blocks is a pathogenetically justified and effective therapy of SHP.
