Comparison of embryo transfer outcomes in patients of advanced reproductive age with and without AI use

The purpose — to evaluate the impact of artificial intelligence (AI) technologies in embryo selection on the outcomes of in vitro fertilization (IVF) cycles in women of advanced maternal age.
Material and methods. The prospective study included 60 patients over 35 years of age with preserved ovarian reserve undergoing IVF with elective single blastocyst transfer from January 1, 2024, to April 1, 2024, at “Semya” Medical Center in Ufa. Two groups were formed by randomization: in the study group, embryo selection was performed using the AI platform ERICA 1.0; in the control group, selection was based on Gardner morphological criteria. Clinical pregnancy rate and pregnancy continuation up to 22 weeks of gestation were assessed.
Results. No significant differences in clinical pregnancy rate were found between the groups; however, pregnancy continuation up to 22 weeks was observed more frequently when AI was used for embryo assessment (p < 0.05).
Conclusions. Application of AI algorithms for embryo selection in women of advanced reproductive age did not demonstrate a statistically significant increase in clinical pregnancy rate but was associated with a significant increase in the probability of pregnancy continuation. This suggests the technology’s potential ability to identify embryos with higher euploid potential and viability, which may lead to improved efficacy of assisted reproductive technology programs.
