Comparative features of electrophysiological remodeling of myocardium in patients with rheumatoid arthritis in combination with arterial hypertension

Patients with rheumatoid arthritis have an increased risk of cardiovascular complications, including due to myocardial injury. Many diagnostic methods are built on the detection of structural geometrical remodeling, whereas at the early stages it is preceded by the electrophysiological remodeling of the heart. One of the available diagnostic methods is vectorcardiography based on the principle of obtaining a spatial image, which is a graphical representation of the changes in the magnitude and direction of electromotive force during the entire cardiac cycle. Our study involved 124 patients with rheumatoid arthritis with or without arterial hypertension. They underwent vectorcardiography to determine the area of loops P, QRS, T, and the maximum vector (MV), MV-azimuth and MV-ascent. When comparing groups of patients with rheumatoid arthritis with or without arterial hypertension, we found that the presence of hypertension significantly increases the value of the area of QRS loop (p<0.05) compared to its absence. This suggests that patients with rheumatoid arthritis and arterial hypertension, in which echocardiography revealed no myocardial hypertrophy, demonstrate the electrophysiological remodeling of the left ventricle.
