Objective ― to conduct a comparative analysis and improve the results of treatment of patients with surgical pathology of the parathyroid glands through adequate examination and preoperative preparation, choosing the tactics of surgical intervention and rational postoperative management.

Material and methods. A comparative description of patients with primary hyperparathyroidism was conducted between 2002 and 2018 (32 patients, the first group) and between 2018 and 2019 (24 patients, the second group).

Results. A sharp increase in the incidence of this pathology has been identified at the Center for Liver and Endocrine Surgery of the Perm Territory since 2018.

Conclusions. An analysis of the results showed the following distinguishing features: prevalence of older patients, comorbidities and a mild form of PHPT in the second group. An improvement in the diagnostic quality of primary care physicians was noted.
