The purpose — to present the data of a clinical case of non-compact left ventricular myocardium complicated by severe cardiac arrhythmias and heart failure.

Material and methods. The diagnosis of non-compact myocardium of the left ventricle became possible since 1990s after the introduction of echocardiography as a diagnostic method. The disease genesis is still unclear, starting with clinical manifestations of complications: cardiac arrhythmia or heart failure or thromboembolism, leading to fatal outcomes.

Conclusions. The article is of interest to practitioners, since non-compact myocardium of the left ventricle is a rare disease, which presumably has both a «family» and a «non-family» character, with the possible occurrence of a triad of syndromes or an asymptomatic course without sufficient evidence for the treatment of this pathology.
