Capabilities of daily monitoring of arterial pressure and tests with dosed physical load in diagnostics of arterial hypertension in young athletes

Objective ― to study the level of arterial pressure and determine the incidence of arterial hypertension in athletes younger than 16 y.o.
Material and methods. 82 young athletes were examined, with the average sports experience of 6.7±2.1 years, and 50 healthy children of the control group. The examination included echocardiography with color Doppler mapping and determining the mass index of the left ventricle myocardium, daily monitoring of arterial pressure (DMAP), test with dosed physical load (DPhL) on a bicycle ergometer; the results were processes with standard methods of variation statistics.
Results. In the routine measurement, AP increase was marked in 1.2% of young athletes. DMAP revealed insufficient decrease of AP at night and high variability of systolic AP in 3.6% of athletes, while arterial hypertension was diagnosed in 7.1% of athletes (of them in 66.6% ― labile form), who had normal values of AP in routine measurement. DPhL test revealed changes in AP even in young athletes with normal results of DMAP: hypertensive type of reaction to physical load in 10 athletes (12.2%), of which only three had a circadian profile of “non-dipper” type by DMAP results. A test with dosed physical load showed that the 95 centile of AP level in DPhL test for 14-15 y.o. was 221 Mmhg. Increase of this indicator during DPhL test was marked in 12.2% of athletes.
Conclusions. DMAP and DPhL test are more informative for diagnosing arterial hypertension, than a routine office measurement of AP. It is expedient to recommend including the test with dosed physical load into obligatory examinations of young athletes in combination with DMAP.
