Abstract. The review presents current literature data on the anti-angiogenic therapy of macular disorders. Information on all existing anti-VEGF agents is given with an emphasis on Ranibizumab and Aflibercept registered in Russia. Different treatment regimens are considered, including the most common in everyday practice, “as needed” (Pro Re Nata, PRN) and “treat-and-extend” (T&E) with an analysis of the advantages and disadvantages of each. The regime that allows to achieve optimal treatment results with the minimum number of injections and visits must be effective, proactive, individualized and convenient. The closest to these criteria today is T&E. The most important task is to approximate the results obtained in real clinical practice to the data of clinical studies. To improve the direction, further clinical research (including real practice) is needed, as well as the study of life quality and adherence of patients to treatment.
Key words: anti-VEGF therapy, regimen, fixed mode, “as needed” (PRN), “treat-and-extend” (T&E).
