A new diagnostic method for early detection of perinatal optic nerve atrophy

The effectiveness of the developed diagnostic method for early detection of perinatal optic nerve atrophy (ONA) was analyzed.
139 children (278 eyes) were under dynamic observation within one year, who had ischemic-hypoxic preconditions for development of perinatal ONA in the obstetrical anamnesis. Preclinical diagnosis of ONA was performed at the age of 2 months on the basis of hemodynamic monitoring results summarized in the regional ischemic-hypoxic index (IHI). At the age of 6 months all 139 children underwent electrophysiological diagnostics to verify the diagnostic level of IHI in a prospective study.
The deviations of regional hemodynamics were diagnosed in 39 children (28%) of the total number of the examined children. Clarifying electrophysiological diagnostics confirmed the presence of changes, characteristic for ONA, in 25.2% of patients. It was found that in presence of electrophysiological signs of ONA in children, the IHI exceeded 1.5 c.u.
The developed algorithm scheme based on a phased hemodynamic and electrophysiological examination of 139 children with perinatal ischemic-hypoxic complication allows diagnosing a preclinical stage of perinatal ONA in the first months of their life with a probability of more than 90%.
