Tonometry of patients with primary open-angle glaucoma taking into account the individual morphometric eye parameters and biomechanical properties of corneoscleral shell

To analyze the correlation between the eye morphometric parameters and tonometry data, taking into account the viscoelastic properties of the cornea in patients with primary open-angle glaucoma (POAG) with initial and advanced disease stages.
Material and methods. 51 patients (99 eyes) aged 64,51±0,98 years on average with the initial – 45 (45,5%) eyes and advanced stages – 54 (54,5%) eyes of the POAG disease were examined at the Optimed Laser Center of Vision Recovery in Ufa. The control group consisted of 31 (62 eyes) healthy people without ophthalmic pathology. In addition to standard ophthalmological examination methods, special research methods were used, such as ultrasound echobiometry to measure the anterior-posterior distance of the eye, pachymetry, and also tonometry using the Ocular Response Analyzer (ORA, Reichert Inc., USA).
Results. The main tonometry parameters of ORA: corneal-compensated intraocular pressure (IOP), corneal hysteresis and the corneal resistance factor in the eyes with the initial and advanced stages of POAG, showed significant differences in comparison with the parameters of eyes without glaucoma. A moderate positive correlation was found between the central corneal thickness and the level of IOP in contactless tonometry, tonometry by Maklakov technique, and the corneal resistance factor. Moderate positive correlation with corneal hysteresis was found at the initial stage of POAG, which was not observed at the advanced stage of the disease.
The conclusion. The state of biomechanical properties of the cornea and its thickness are relevant parameters while interpreting IOP level in POAG diagnosis.
