Study objective ― optimization of the algorithm for the therapy of the dry eye syndrome in rheumatoid arthritis.
Methods. The study included 29 patients (58 eyes) aged 50 to 65 years with a proven diagnosis of DES of mild and moderate severity. The patients received treatment at the Rheumatology Department of the North-West State Medical University named after I.I. Mechnikov in connection with rheumatoid arthritis (disease activity score ― DAS28 <3.2). Patients were split into groups: 16 patients (32 eyes) entered the 1st group, these patients instilled Stillavit® 3 times per day for 3 months. This group was split into 2 subgroups: with mild form of DES ― 8 patients (16 eyes) and moderate form ― 8 patients (16 eyes). 13 patients (26 eyes) entered the 2nd group; these patients instilled Eyestill 3 times per day for 3 months. This group was also split into subgroups: with mild form of DES ― 7 patients (14 eyes) and moderate form ― 6 patients (12 eyes). Examination of the patients was conducted on the day of attendance and on the 90th day of the study.
Results. According to the changes in the osmolality of the tear film and the state of the cornea with the use of vital dyes, positive dynamics was observed in the treatment of mild to moderate DES, more pronounced with the use of Stillavit®, as compared with Eyestill.
Effectiveness of both drugs manifested in decrease in edema and hypoxia of epithelial cells and anterior surface of stroma, as well as in decrease in tear film osmolality.
Conclusions. High effectiveness of Stillavit® in treatment of «dry eye» syndrome of mild and moderate severity in patients with RA allows recommending it as a drug of choice, and it extends the list of drugs for practical healthcare.
Key words: rheumatoid arthritis, dry eye syndrome, cornea xerosis, tears film osmolarity, tear substitutes, Stillavit®, Eyestill.
